The local organisation is a charity which is part of a network of 139 Age UK’s across the United Kingdom recognised for quality services, understanding the changing needs of older people and supporting the most vulnerable. You will have overall responsibility for the running of the Fall Stop, Care Coordination for Frailty, Memory Care and iCan Service on a daily basis. In doing so, you will assess, plan, deliver in line with contract requirements. You will be responsible for responding to emerging knowledge on social prescribing, working closely with the Head of Care. Along with managing the community navigators, the post holder will also carry a caseload of clients referred to the service. At an operational level, you will develop good working relationships with GP practices and voluntary sector and community service providers.
