Role: Care Connector Coordinator – Full Time 35 hrs per week Enfield, North London £28,000 per annum 35 hours per week Benefits package includes pension, health insurance, flexible working options, 25 days annual leave This is an exciting opportunity to lead a new, pioneering programme, which is at the heart of Enfield’s prevention and early intervention strategy. The Care Coordinator is a partnership between the Enfield CCG and Age UK Enfield. The Service funded through Enfield CCG will focus on supporting older people who have a diagnosis of ‘frailty’ including the following indicators: • Falls, Immobility, Delirium, Incontinence and susceptibility to side effects of medication The Care Co-ordinator will provide critical links between formal health and social care services and the wider community support networks to provide a single point of access. The role will involve case management through a patient pathway. The important qualities we are looking for are; • Knowledgeable of prevention services • Experience of working in partnership across the voluntary sector, integrated care networks and GP’s • Committed to person centred working • Committed to transforming the lives of older people and their carers Main duties of the role • Be a participatory member and embedded within the GP multidisciplinary team (MDT) and Integrated Locality Team (ILT) • Overseeing co design and facilitation of care plans • Conducting ongoing assessments and setting up regular home visits • Liaising with services and attending case conferences. • Carry out assessment for: falls, loneliness, nutrition and well being, assistive • technology • Signpost and follow through to arrange access to services from multiple organisations • Evaluation of the pilot programme including outcomes to ascertain future funding for the programme To request a job pack please email: [email protected] For an informal discussion please contact Venetta Hunt, Head of Care Services on 020 8351 1131.
